Introduction
Patient demographics can be updated at any time to keep records current. This includes personal details such as name, date of birth, contact information, and identification numbers. Keeping patient information accurate ensures proper care coordination and communication.
Before You Start
- You must have access to view the patient's profile
- Any team member who can view a patient can update their demographics
- Changes are logged in the patient's audit trail
Steps

From the sidebar, select My Patients to view your patient list.

Click on the patient whose details you need to update. The patient's profile page will open.

At the top of the patient profile, click the kebab menu (⋮) in the top right corner.

Select Edit Demographics from the dropdown menu. A dialog box will appear showing the patient's current information.

Update any of the following fields as needed:
- First Name (required)
- Last Name (required)
- Date of birth (optional)
- Gender (optional - select from Male, Female, or Other)
- Nat.ID (optional - national ID number)
- Medical Record Number (optional)
- Email (optional)
- Cell / Mobile number (optional)

Click the Submit button at the bottom of the dialog.

The dialog will close automatically when the update is successful. You'll see the updated information reflected on the patient's profile page immediately.
Tips

Required fields: First Name and Last Name must always be provided. All other fields are optional.

Date of birth validation: If you begin entering a date of birth, you must complete all three fields (day, month, year). Partial dates cannot be saved.

Email validation: Email addresses must be in a valid format (e.g., name@example.com). The system will alert you if the format is incorrect.

Changes are logged: All demographic updates are recorded in the patient's Log tab, showing who made the change and when.

Contact details: Updating a patient's email or mobile number here does not send verification messages to the patient - these are informational fields for care coordination purposes.

Last name display: Throughout CAREFUL, patient last names are displayed in uppercase (e.g., John DOE) to make them easily identifiable.
