Introduction
Taking responsibility for a patient assigns you as their primary contact within a team. This helps colleagues know who to contact about the patient's care and ensures clear accountability during shifts and handovers.
Before You Start
- You must be a member of at least one team that the patient is assigned to
- You cannot already be responsible for the patient
- No special permissions are required beyond team membership
Steps

From the sidebar, select My Patients to view your patient list, or select a team name to view that team's patients.

Click on the patient row to open their patient profile.

In the top-right corner of the patient view, click the three-dot menu (⋮) to open the context menu.

Click Take Responsibility from the menu.

The action executes immediately without a confirmation dialog. You'll see the following changes:
- A My Patient badge appears on the patient's profile header
- Your name is added to the responsible users list
- The patient now appears in your My Patients list (if they weren't already there)
- An audit log entry is created: "Responsibility for patient taken"

The kebab menu options update automatically:
- Take Responsibility is removed
- Relinquish responsibility and Handover Patient options become available
Tips

No confirmation required – Taking responsibility is a single-click action. The system assumes you intend to take responsibility when you click the menu item.

Notifications sent – If other users were previously responsible for this patient and are currently on duty, they receive a push notification: "[Your name] has taken responsibility for one of your patients."

Team membership matters – You can only take responsibility for patients in teams where you are a member. If you don't see the Take Responsibility option, check that you and the patient share at least one team.

Already responsible? – If you're already responsible for the patient, the Take Responsibility option won't appear in the kebab menu. Instead, you'll see Relinquish responsibility and Handover Patient options.

Audit trail – All responsibility changes are recorded in the patient's Log tab with timestamps and user details for compliance and handover tracking.

Bulk responsibility – To take responsibility for all patients in a team at once, navigate to the team view and use the kebab menu option Take responsibility for all patients.
